ESSENTIAL FUNCTIONS / SUMMARY

• The DS Service Coordinator performs duties of a Qualified Developmental Disabilities Professional, as assigned by supervisor.
• Screening to evaluate consumer strengths and needs.
• Intake and enrollment, including consumer and family orientation with individualized program planning and service development.
• Service coordination, utilization review, crisis intervention, referral and consultation.
• Quality assurance, reporting and record keeping.
• Maintain strict confidentiality per Federal Confidentiality Regulations (42 CFR Part
  2) and NKHS policies.
• Resource development to secure required services and screen, supervise and monitor contracted providers and supportive living arrangements.
• Ensure housing safety requirements are met for contracted homes.
