POSITION CONCEPT: CWI is seeking dedicated Community Support Professionals to provide Community Habilitation and Respite services throughout Warren, Washington and Northern Saratoga Counties.
Community Habilitation supports individuals with intellectual and developmental disabilities, build independence and become active members of their communities. Everyday activities like grocery shopping or going to the movies can require support, planning and creativity. Community Support Professionals assist individuals in navigating these experiences safely and meaningfully.

Professional. We offer flexible scheduling based on your availability and the needs of the individuals we support. We support individuals with a wide range of availability and can match staff with individuals whose availability align with their own.
Our Community Support Professionals:
- Work independently, with supervision and support from the Community Services Team.
- Provide person-centered services aligned with individualized Staff Action Plans, which reflect each individual’s goals, strengths, and needs.
- Support, supervise, and guide individuals as they pursue their chosen goals.
- Use the electronic health records system to document services.
- Attend occasional meetings and in-service training as required.
- Interact with individuals in a therapeutic manner to promote social and interpersonal development.
- Model positive, professional behavior in interactions with community members and other professionals.
- Deliver flexible, adaptive, and appropriate care and supervision to the individuals they support including assistance with daily living activities, community outings and social engagements.
We are seeking ambitious, responsible, caring people who can maintain professionalism while helping individuals with disabilities lead fuller lives. A high school diploma is preferred.
Community Support Professionals must:
- Possess good character and pass a standard background check.
- Hold a valid driver’s license and an acceptable driving record.
- Successfully complete training in First Aid/CPR and safety procedures.
- Be able to work independently.
- Have at least one year of experience working with people with disabilities.
