LTSS Service Coordinator-Clinician
Location: Field based. Candidate must reside in one of the following counties:
Collier, De Soto, Glades, Hendry, Lee, Pasco, Pinellas, Sarasota.
Field: This role primarily operates in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training.
Schedule: Monday-Friday 8am-5pm EST.
How you will make an impact- Assist the responsible RN in identifying members for high‑risk complications.
- Obtain clinical data as directed by the responsible RN.
- Assist the RN in identifying members that would benefit from alternative levels of care or other waiver programs.
- Provide collected information to the RN for verification and interpretation, develop, monitor, evaluate, and revise the member’s care plan.
- Participate in coordinating care for members with chronic illnesses, comorbidities, or disabilities, ensuring cost‑effective and efficient utilization of health benefits.
- Use decision‑making skills based on current member needs, applying disease knowledge, terminology, and clinical guidelines. Nursing judgment is not required.
- LPN/LVN, LSW, LCSW, LMSW, or comparable license in accordance with state law, and a Nursing diploma or AS in Nursing or related field.
- At least 2 years of experience working with individuals with chronic illnesses, comorbidities, or disabilities in a Service Coordinator or similar role, or equivalent combination of education and experience.
- Current, unrestricted license (LPN/LVN, LSW, LCSW, LMSW, or other non‑RN license as allowed by state law).
- Bilingual in Spanish highly preferred.
- Long‑term care experience preferred.
- MA/MS in Health/Nursing preferred.
- May require state‑specified certification based on state law and/or contract.
- Willingness to travel to worksite and other locations as necessary.
