Home Health LPN (PRN) - Empath Home Health - Venice, South Sarasota, Nokomis, Laurel, Siesta Key

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health LPN to provide skilled nursing care for patients in their homes throughout the Venice, South Sarasota, Nokomis, Laurel, Siesta Key, and surrounding areas
. In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Locations: Home health visits in the Venice, South Sarasota, Nokomis, Laurel, Siesta Key, and surrounding areas

Schedule
: PRN; 5-15+ home health visits available per week

• Provides nursing services in compliance with agency policy and procedure, state and federal regulations, and the patient’s plan of care under the direction and supervision of a Registered Nurse.
• Observes patients and report adverse reactions to medication or treatment to the Clinical Coordinator.
• Assists the Physician and/or Registered Nurse in carrying out the plan of care and in performing specialized procedures.
• Performs patient and family instruction on diet, medication, and treatment.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

• Active Florida LPN license
• Applicants must be a graduate of an approved nursing program.
• Minimum of 1-2 years of experience as a LPN.
• Home health care experience required.
• Strongly prefers experience using Homecare Homebase (HCHB) EMR
• Intravenous (IV) Certification preferred.
