Home Care Supervisor - 3 month contract

- Full-time 3-month contract (February to May 2026) with possibility of extension

- Hybrid work involving community and office
- Supervise and support Personal Support Workers and Nursing staff; this will include training, monitoring, and evaluation.
- Create and review care plans during client/patient visits, support staff and update care plans as required.
- Provide delegation and training to Personal Support staff as required based on client’s needs, with ongoing evaluation of efficacy of training.
- Liaise with Care Teams and families as required.
- Conduct home safety assessments on patient/client admission and reassess for safety/risk issues at each home visit.
- Conduct incident reports and investigations as required.
- Participate in rotating on-call schedule.
- Participate in educational sessions.
- Conduct additional tasks as required/assigned.
- Diploma or degree in Nursing
- Minimum of one (1) year of related work experience.
- Registered and in good standing with the College of Ontario
- RNAO or RPNAO me
- Ability to maintain professionalism and confidentiality to cope with emergencies, ongoing client care and employee management.
- Excellent understanding of the healthcare services and delivery in the community
- Strong leadership skills
- Strong critical thinking, problem-solving and organizational skills.
- Ability to work independently.
- Excellent customer service and communication skills (verbal and written)
- Previous experience with Microsoft Office and web-based applications
- Valid license and reliable vehicle (mandatory)
- Working understanding on LHIN structure in the community (preferred)
- Previous experience in supervisory roles in a community setting (preferred)
- Experience as a Home Supervisor, PSW Supervisor or Community Supervisor considered an asset.
