Overview

As an HD Nurse, you'll be at the forefront of patient care, working collaboratively within an interdisciplinary team to ensure patients/residents receive comprehensive support.

Responsibilities
  • Assess patients' physical, social, spiritual, and psychological needs
  • Develop, implement, and evaluate individualized care plans
  • Incorporate teaching into care plans to educate patients and families
  • Make referrals to resources as needed
  • Support fellow healthcare professionals
  • Operate within the framework of Providence Health Care's mission, vision, and values
  • Hemodialysis RNs care for patients with kidney failure undergoing dialysis treatment
  • Focus on ensuring patients receive safe, effective, and compassionate care throughout their treatment
  • Conduct thorough patient/resident assessments through chart reviews, observations, interviews, and collaboration with peers
  • Develop, implement, and evaluate nursing care plans based on evidence-based practices, prioritizing patient/resident needs
  • Adhere to established policies, procedures, and PHC standards while providing high-quality nursing care
  • Monitor patient progress, evaluate outcomes, and adjust care plans as needed for optimal results
  • Maintain accurate records of assessments, care plans, patient/resident status, and discharge plans using manual and computerized systems
  • Identify and address patient/resident and family learning needs, providing education and referrals as necessary
  • Provide direction to staff, promote teamwork, and share clinical knowledge to enhance patient care
  • Support nursing research initiatives and contribute to the improvement of patient/resident care policies and procedures
  • Advocate for patients/residents  rights and dignity, facilitate discharge plans, and collaborate with families and community resources
  • Perform additional duties as assigned to support the healthcare team
Education
  • You must be a registered nurse in Canada in good standing and be eligible for registration with the BC College of Nurses and Midwives (BCCNM).
Skills and Abilities
  • Demonstrated competence in performing CPR procedures
  • Experience in teamwork, decision-making and conflict resolution
  • Demonstrated a teaching aptitude
  • Proficient in clinical leadership
