The salary range for this position is CAD $59.56/Hr. – CAD $85.62/Hr.

Foundry is transforming health care for young people ages 12‑24 by removing barriers, improving access to services, and increasing the quality of supports. It’s a made‑in‑BC health and social service model designed by young people, for young people.

Beginning with a single location in Vancouver on Granville St., Foundry has expanded into a world‑renowned collaborative network of over 17 centres open across the province, with more locations in development.

Owned and operated by Providence Health Care, the Foundry Granville Clinic provides physical & sexual health, mental health, substance use, peer support and social services via a multi‑disciplinary team to young people between the ages of 12 and 24. The Nurse Practitioner at the Foundry Granville Clinic provides episodic, enhanced/specialized and longitudinal care. The collaborative team includes physicians, nurse practitioners, registered nurses, social workers, counsellors, occupational/rehab services, peer support, and administrative staff.

At the Foundry Granville Clinic, Nurse Practitioners also have access to shared care consultation with the psychiatrists on site.

Responsibilities
  • Collaborating with patients and healthcare professionals to identify trends and patterns affecting individuals, families, and communities.
  • Developing and implementing population and evidence‑based strategies to improve health outcomes.
  • Participating in policy‑making activities influencing health services and practices.

Engaging in peer review and self‑review to evaluate service outcomes at patient, community, and population levels.

Note we have two part‑time positions available (0.50 FTE and 0.80 FTE).

Education
  • Current registration as a Nurse Practitioner with the British Columbia College of Nursing Professionals and recent relevant clinical nursing experience.
  • Completion of the Basic Cardiac Life Support Course, Level

    C.
  • Eligible for Appointment to the Providence Health Care Department of Nurse Practitioners as Medical Staff.
Skills and Abilities
  • Ability to communicate and collaborate with patients/clients/residents and families about health findings, diagnosis, treatment, self care and prognosis.
  • Ability to collaborate, consult with and formally refer patients/clients/residents to physicians and other health professionals when appropriate.
  • Ability to critically assess and evaluate health research literature to determine best practices; ability to introduce education and evidence‑based research.
  • Ability to assess and recognize population health trends; ability to plan and implement strategies for population‑based prevention and health promotion.
  • Ability to implement and evaluate planned change.
  • Ability to supervise others and evaluate the care they deliver.
  • Ability to lead a team and work within a team.
  • Ability to self‑direct, interact, and adapt effectively with other professionals in complex, dynamic situations.
  • Ability to transfer knowledge, teach, coach and mentor others.
  • Ability to identify and respond appropriately to legal and ethical issues that may arise in patient/resident/client care.
  • Ability to self‑assess performance and assume responsibility and accountability for own professional development, educational or consultative assistance when appropriate.
