Reporting to the Program Coordinator or designate and under the direction of the team leader, the Licensed Practical Nurse (LPN) Outreach works as a member of the multidisciplinary mental health services team in a collaborative nursing model with RNs and RPNs. The LPN Outreach performs assessments and plans, implements and provides personal and nursing care services to clients, both in their homes and in the community, who have a variety of mental health disorders.
The LPN Outreach operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM) and in conformance with established policies, procedures, standards and philosophies of Fraser Health.
Education and Experience
Graduation from a recognized program of Practical Nursing or an equivalent combination of education, training and experience.
Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Current First Aid, CPR, and Food Safe certificates.
Completion of or eligible to complete MHSU Competency Education.
Valid BC Driver's License and access to a personal vehicle with appropriate insurance coverage.
