As a Social Worker with Vancouver Coastal Health, you will:
- Work as part of an interdisciplinary team to provide outreach services to individuals in the community who have difficulty accessing services with serious substance use disorders and other complex medical and psych-social needs such as mental health and/or primary care needs.
- Provide comprehensive Social Work assessments, connects clients with health care and community services, develops, implements and adjusts individualized treatment plans in collaboration with internal team, affiliated prescribers and other treatment team members and provides counselling according to a variety of models.
- Participate in team case conferences/meetings to coordinate and manage client care issues, aids clients access to needed services, establish and maintain relationships, liaises with staff of emergency rooms, inpatient psychiatric units and other resources and collaborates and consults with colleagues. Participates in team meetings, continuing education, conference and/or corporate initiatives and engages in on-going team planning, development, evaluation and quality improvement activities.
- Provide clinical and consultative Social Work services, including outreach and brief counselling.
- May act as a Designated Responder under the Adult Guardianship Act.
Our team of outreach and social workers and peers provide connections for people who have recently experienced opioid overdose and/or are at high risk for opioid overdose to substance use care and support.
QualificationsEducation & Experience
- Current full registration with BC College of Social Workers.
- Master’s Degree in Social Work from an accredited educational institution.
- Two (2) years’ recent related clinical experience working with individuals with substance use, mental health and/or primary care needs in community-based settings or Indigenous-led/First Nations organization, or an equivalent combination of education, training and experience.
- Valid BC Driver’s License as local area travel requires the use of a vehicle.
Knowledge & Abilities
- Broad knowledge of theories and practices within a client and family centred care model.
- Broad knowledge of crisis intervention and counselling skills.
- Demonstrated ability to conduct psychosocial assessments.
- Demonstrated ability to provide treatment planning, counselling, crisis intervention, and case coordination.
- Broad knowledge of substance use and mental health diagnoses and treatment.
- Broad knowledge of substance use treatment and harm reduction modalities.
- Knowledge and experience in providing culturally safe and competent care, specifically pertaining to Indigenous people.
- Demonstrated experience working with marginalized populations including experience working with Indigenous people.
- Working knowledge of Indigenous-specific cultural interventions for mental health and substance use including ceremonial practices and land-based healing.
- Working knowledge and experience in Trauma Informed Practice, Harm Reduction and Recovery Oriented Care.
- Knowledge of appropriate acts; e.g. Mental Health Act, Adult Guardianship, Public Health Act as it relates to the rights and obligations of clients and staff.
- Broad knowledge of psychopharmacology (indication and side effects).
- Broad knowledge of other facilities and community resources.
- Demonstrated ability to communicate effectively, both orally and in writing, with clients and their families, colleagues, physicians and other health care staff, both one-on-one and in groups.
- Demonstrated listening and information seeking skills that promotes communication and leads to a cooperative approach to problem solving within an interdisciplinary setting.
