Job Description:
Collaborates with the interdisciplinary team, the patient/family/caregiver, and the patient’s primary care provider to develop a plan of care focused on achieving the patient/family physical therapy goals.
Instructs, coaches, guides, and intervenes to support the patient and family in achieving the best possible outcomes while remaining in the communities they value.
Promotes optimal health, well-being.
Assesses the patient and collaborates with the interdisciplinary team and primary care provider to develop the patient-centered plan of care.
Provides physical therapy treatment to the patient/family/caregiver as ordered.
Completes accurate clinical documentation to effectively support service provided within specified time frames
Provides physical therapy to pediatric, adolescent, adult and geriatric patients based on principles of growth and development and life stages.
Provides instruction to the patient/caregiver and revises the treatment plan as appropriate.
Maintains communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
Reports acute medical problems and condition changes to the primary care provider.
Completes regulatory required assessments within specified time frames.
Completes medication reconciliation at specified time frames.
Qualifications:
Must meet all state educational requirements
1+ years of experience
Current license in Physical Therapy or ability to obtain prior to hire.
American Heart Association Health Care Provider BLS preferred upon hire, and required within 90 days of hire.
Excellent customer service skills
Ability to work without direct supervision.
Provides care appropriate to the age of patients served on the job.
Knowledge of physical therapy assessment and treatment methods
Knowledge of physical therapy modalities and equipment
Ability to maintain effective relationships with other health care professionals
Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
