Compensation- Base pay range: $ – $ per year.
- Annual bonus: up to $10,000 (paid quarterly).
- Additional promotion to PTII with a minimum salary of $135,000 plus $20,000 quarterly bonus.
- New‑hire incentives for start dates in November 2025.

Responsibilities- Perform evaluations in patients’ homes, develop individualized care plans, and collaborate with PTAs to implement plans.
- Assist patients with mobility, movement, and independence restoration.
- Create detailed treatment and care plans for each patient.
- Coordinate with physicians to ensure quality care.
- Evaluate and report patients’ progress to the MD.
- Supervise Physical Therapy Assistants (PTAs) per agency and regulatory standards.
- Concierge service: assistance with documentation and administrative tasks.

- Current unrestricted California Physical Therapist license.
- Current BLS/CPR certification.
- Valid California driver’s license.
- Knowledge of local community resources.
- Understanding of health care delivery and cultural influences on health.
- Knowledge of state and federal home‑care regulatory guidelines (preferred).
- Experience in home health care (highly preferred).
- Experience with high‑acuity patients (plus).
- Experience working with interdisciplinary teams (highly desired).
- Case management fundamentals (highly desired).
- Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership, organized.
