We are open to newer LMSWs as long as they are comfortable working in a hospital/unit setting and managing a high-volume caseload. Candidates with prior hospital experience are strongly preferred.

  • Conduct psychosocial assessments for hospitalized patients
  • Manage discharge planning for an assigned caseload of approximately 25 patients per day
  • Coordinate post-acute services, community resources, and follow-up care
  • Collaborate daily with Nurse Case Managers, physicians, nurses, and interdisciplinary team members
  • Address barriers to discharge including housing, insurance, transportation, and social support needs
  • Provide emotional support, counseling, and education to patients and families
  • Ensure documentation is completed accurately and timely in accordance with hospital policies
  • Advocate for patient needs while maintaining compliance with regulatory and ethical standards
Required Qualifications
  • Licensed Master Social Worker (LMSW) – required
  • Comfortable working in a hospital/unit-based setting
  • Ability to manage a fast-paced workload and high patient volume
  • Strong communication, organization, and care coordination skills
Schedule
  • Monday–Friday
  • 8:30 AM – 4:30 PM or 9:00 AM – 5:00 PM
  • Inpatient hospital/unit setting
  • High collaboration with clinical staff
  • Patient-centered, team-oriented care model
