We are seeking a compassionate and motivated Medical Social Worker to join a hospital-based care team. This role is ideal for a licensed social worker who thrives in a fast-paced inpatient/unit setting and is passionate about supporting patients through effective discharge planning and care coordination.
Position OverviewThe Medical Social Worker will work directly within a hospital unit, managing a daily caseload of approximately 25 patients
. In this role, you will collaborate closely with a Nurse Case Manager (NCM) and the broader multidisciplinary care team to ensure safe, timely, and appropriate discharges. This is a hands-on, patient-facing position within a hospital environment —
not an office or outpatient setting
.
We are open to newer LMSWs as long as they are comfortable working in a hospital/unit setting and managing a high-volume caseload. Candidates with prior hospital experience are strongly preferred.
Key Responsibilities- Conduct psychosocial assessments for hospitalized patients
- Manage discharge planning for an assigned caseload of approximately 25 patients per day
- Coordinate post-acute services, community resources, and follow-up care
- Collaborate daily with Nurse Case Managers, physicians, nurses, and interdisciplinary team members
- Address barriers to discharge including housing, insurance, transportation, and social support needs
- Provide emotional support, counseling, and education to patients and families
- Ensure documentation is completed accurately and timely in accordance with hospital policies
- Advocate for patient needs while maintaining compliance with regulatory and ethical standards
- Licensed Master Social Worker (LMSW) – required
- Comfortable working in a hospital/unit-based setting
- Ability to manage a fast-paced workload and high patient volume
- Strong communication, organization, and care coordination skills
- Monday–Friday
- 8:30 AM – 4:30 PM or 9:00 AM – 5:00 PM
- Inpatient hospital/unit setting
