We are seeking an experienced Medical Social Worker to perform advanced clinical evaluations and provide treatment to individuals in need of medical or mental health services within a healthcare setting. This role carries significant clinical responsibility and involves independent clinical judgment while operating within established policies and procedures.
The position primarily supports a pediatric population, with the opportunity to transition to an adult team if preferred. The Medical Social Worker serves as a clinical resource to interdisciplinary teams and provides expert consultation on complex psychosocial issues related to illness, injury, and treatment planning. While this role does not include formal supervisory responsibilities, leadership and training of social work students and entry-level staff are core components.
Key Responsibilities- Conduct comprehensive psychosocial assessments of patients and families and present findings to interdisciplinary teams
- Provide psychosocial counseling with minimal supervision to support patient and family adjustment to illness, injury, and treatment plans
- Coordinate and, on certain units, oversee discharge planning, including referrals to home health services, residential facilities, rehabilitation programs, and community resources
- Participate in interdisciplinary rounds, case conferences, and treatment planning meetings
- Provide in-service education and training to social work staff and other clinical disciplines
- Train and provide clinical leadership to social work students and entry-level social workers on assigned units
- Complete required documentation, statistics, and records for the Social Work Department
- Stay current with developments in medical social work and areas of clinical specialization
- Represent the organization at community meetings and act as a liaison between hospital and community programs as needed
- Minimum of two years of professional social work experience in a medical, mental health, or human services setting as part of a multidisciplinary treatment team
- Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education
- Current and valid LMSW or LCSW license issued by the New York State Education Department
- License and registration must be maintained throughout employment
- Strong knowledge of medical social work principles and practices
- Understanding of disease processes and their psychosocial impact on patients and families
- Knowledge of medical treatments, technology, and care modalities
- Advanced casework, assessment, crisis intervention, and diagnostic skills
- Ability to work effectively with patients and families facing serious illness, trauma, or complex medical conditions
- Strong written and verbal communication skills
- Ability to collaborate effectively within interdisciplinary healthcare teams
- Leadership ability to train and mentor social work staff and students
- Proficiency with standard computer applications and electronic documentation systems
- Demonstrated professional judgment, initiative, integrity, and emotional maturity
