We are Health Flex Home Health & Hospice, one of the nation’s largest privately‑owned agencies committed to delivering exceptional care in the comfort of home across the San Francisco Bay Area, North Bay, Central Valley, and the Greater Sacramento Area. We are looking for skilled, empathetic LVNs to join our team.

Position Summary: LVN Home Health professionals conduct patient assessments, administer medications, monitor vital signs, educate patients and families on health management, maintain accurate records, and provide ongoing support. You work collaboratively under RN supervision, utilizing strong clinical skills and a compassionate approach.

Key Responsibilities:

• Conduct patient assessments and develop care plans.
• Administer medications and document administration accurately.
• Monitor patient health status and respond to changes.
• Educate patients and families on disease management and self‑care.
• Maintain up-to-date nursing records and comply with quality standards.
• Collaborate with RNs, case managers, and interdisciplinary team members.

Pay Range: $90,000 – $110,000 per year, based on experience, skills, location, and market data.

Requirements:

• Current and unrestricted California LVN license.
• Minimum one year of direct patient care experience as an LVN.
• Current AHA CPR certification.
• Valid driver’s license.
