Overview
Anchor Co-Living | CalAIM Program Anchor Co-Living is seeking a Lead Care Manager to support Medi-Cal members through CalAIM by providing care coordination and housing navigation services.
Responsibilities- Assess member medical, behavioral, social, and housing needs
- Create and manage individualized care plans
- Coordinate healthcare, social services, and housing supports
- Assist with housing applications, move-ins, and tenancy support
- Build relationships with landlords and housing agencies
- Maintain compliant documentation and reporting
- High school diploma or GED required; degree preferred
- 1+ year experience in care coordination, case management, or housing navigation
- Familiarity with CalAIM, Medi-Cal, and housing systems preferred
- Strong communication, documentation, and advocacy skills
- Valid CA driver's license and reliable transportation
$21-$25 per hour, based on experience
Work Environment- Hybrid role with remote, and in-person member visits
