Overview
What you’ll do as a Home Health Speech Therapist/Language Pathologist (ST):
In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team.

Base pay range: $/yr - $/yr
Location requirements:
This opportunity is a fully in-person, home health role. Remote or hybrid work is not available. You must be located in, or willing to relocate to, the specified territory (e.g., Solano County: Vacaville or nearby areas).
- Provide skilled services to assist patients with breathing, speaking, and swallowing functions.
- Evaluate and treat speech, language, cognitive and fluency disorders that affect communication.
- Serve as an educator, patient advocate, and member of the interdisciplinary care team.
- Current unrestricted CA Speech-Language Pathologist license
- Current BLS/CPR certification
- Valid CA driver’s license
- Knowledge of development and behavior as it relates to illness/injury, and cultural influences on health care
- Knowledge of state and federal homecare regulatory guidelines (preferred)
- Home health care experience (highly preferred)
- Experience with high-acuity patients (a plus)
- Experience with interdisciplinary teams and case management fundamentals (highly desirable)
- Self-directed, empathetic, critical thinker with strong interpersonal and organizational skills
- Prior to scheduling an in-person interview, candidates may complete a computer literacy assessment to ensure readiness for electronic charting and web-based tools.
