Job Summary

The post holder will be responsible for providing efficient and high-quality clerical support to Your Life line 24/7 Palliative and End of Life team.
The service provides telephone triage and clinical visits to patients within the borough of Hillingdon. The administrator will support the service Monday to Friday between 8 am and 4 pm. The post holder will play an integral part of the team to maintain smooth running and operation of the service, working closely with the Team Lead and clinical staff. Excellent interpersonal skills and the ability to communicate effectively with service users, carers, health professionals and team members are required.

The post holder must possess good IT skills, be able to analyse and interpret data, help maintain clinical database systems, and assist with clinical audits and information monitoring. The role requires initiative, flexibility and adaptability to meet changing service needs. Part of the role involves assisting the Clinical Team Leader/Manager with clerical and managerial duties, which involves discretion and confidentiality as the post holder will handle sensitive patient and staff data.

Main duties of the job
YLL is a palliative care service delivering 24/7 care to community patients. The team includes 11 nurses, 2 HCAs and a team lead. You will work alongside another Administrator within YLL 24/7 and the Administrator of the Specialist Palliative Care team to provide administrative support. Triage and admin will handle telephone enquiries from health professionals, service users, service user families, carers and team members in an informed and sympathetic manner, considering mental/physical health needs and cultural or language barriers.

Messages should be taken and passed on promptly and action taken where necessary. The Admin role is vital in supporting the service and clinicians on shift. No two shifts are the same due to varying call volumes and tasks, providing variety in this role within palliative care.

Deal with telephone enquiries from health professionals, service users, service user families, carers and team members in an informed and sympathetic manner, considering mental/physical health needs and cultural or language barriers. Take and pass on messages promptly and take action where necessary. Manage complex and sensitive information with tact and diplomacy. Process new referrals, allocate referrals, complete Excel spreadsheets, and ensure all administrative processes for each referral are adhered to promptly.

Create/accept referrals within the S1 platform and highlight for clinical triage. Assist with onward referrals to other teams. Monitor S1 wait lists and caseloads, assist with patient allocation and entering patient details into relevant diaries. Regularly access and monitor generic email systems, respond to queries or redirect as appropriate. Maintain service line databases for record keeping with accurate reporting. Archive patient notes in line with trust policies.
