Critical Care Outreach Nurse | Hillingdon Hospitals NHS Foundation Trust

The Band 7 senior sister for critical care outreach will be a senior member of the critical care outreach team. They will possess expert clinical skills in the assessment and treatment of acutely unwell and deteriorating patients. They will deliver a high standard of care to patients in ward areas, seeking to avert admissions to Intensive Care or facilitate prompt and safe transfer.

They will assist with safe patient discharge from Intensive Care, providing input into rehabilitation and providing psychological support for patients and relatives. They will provide professional and educational support to colleagues working in other clinical areas. Active participation in audit, quality improvement and research is a key requisite of the role.

The Hillingdon Hospitals NHS Foundation Trust is the only acute hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, a modern treatment centre, a minor injuries unit and outpatient clinics. • 7. In liaison with the multi‑disciplinary team, initiate appropriate diagnostic intervention and accurately interpret and act upon clinical findings

• 9. In collaboration with the multi‑disciplinary team, participate in patient stabilisation prior to and during the transfer process to another clinical area.
• 10. To demonstrate excellent communication skills with all professional colleagues, making appropriate and timely referrals where necessary and working collaboratively to set goals
