Pay: Pay Per Visit including mileage reimbursement

The Home Health – Occupational Therapist will provide direct patient care to home‑bound patients. The therapist will work with patients to develop treatment plans, monitor progress, adjust treatment as needed, and help them achieve greater independence and an improved quality of life. The Occupational Therapist will also collaborate with other healthcare professionals to ensure comprehensive care.

• Develop and revise plan of care in consultation with physician and other care‑team members.
• Provide therapeutic treatments and evaluate equipment needs as applicable.
• Participate in clinical conferences and in‑service programs.
• Conduct on‑site supervisory visits of certified occupational therapy assistants.
• Advise and consult with the family and other agency personnel as applicable.
• Prepare clinical and progress notes.
• Protect and honor patient and coworker confidentiality and the right to privacy.
• Evaluate outcomes of care.
• Plan for discharge.

• Current Texas OT License
• Current Certification in BSL/CPR
• Valid Texas Driver's License and Automobile Insurance
• Knowledge of Home Care Home Base software is a plus but not required
