Een zorgorganisatie in Utrecht zoekt een Persoonlijk begeleider voor het KTC. Je stimuleert de ontwikkeling van cliënten met LVB en biedt hen ondersteuning op maat. Taken omvatten het observeren en begeleiden van cliënten, en het opstellen van zorgplannen. We zoeken iemand met een afgeronde MBO of HBO opleiding in Social Work en enige ervaring met de doelgroep. Flexibiliteit in werktijden is vereist, inclusief onregelmatige diensten.
