Job Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. As an RN in a home health setting, you will provide high-quality care to patients in the comfort of their own homes, ensuring their safety, comfort, and well-being while managing a variety of healthcare needs.
Key Responsibilities:
- Perform comprehensive assessments of patients' health status, including physical, emotional, and social well-being.
- Develop and implement personalized care plans based on the patient's needs, collaborating with other healthcare professionals.
- Administer medications and treatments as prescribed by physicians.
- Monitor and record patients' progress, making adjustments to care plans as necessary.
- Educate patients and families on managing health conditions, medications, and self-care.
- Provide wound care, IV therapy, and other specialized treatments as required.
- Assist in the management of chronic conditions and ensure continuity of care.
- Coordinate with physicians, social workers, physical therapists, and other team members to ensure comprehensive care.
- Valid RN license
- Previous experience in home health or a similar healthcare setting is preferred.
