Key Responsibilities:

Assess resident health conditions and contribute to the development of resident care plans.

Administer medications and treatments as prescribed.

Monitor residents' progress and respond to changes in their condition.

Provide education and health coaching to residents and their families regarding health management.

Communicate effectively with residents, families, and interdisciplinary teams.

Ensure compliance with all safety, infection control, and regulatory guidelines.

Ensures that a safe and effective resident care environment is maintained.

Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team.

Safely administers and documents all forms of medication administration.

Assesses, plans, implements and evaluates care based on age-specific components.

Identify and assess resource utilization and safety issues, taking appropriate action.

Aligns nursing/clinical objectives with organizational strategic outcomes, participating in strategy planning and execution.

Treats all residents, visitors, and staff with courtesy and dignity.

Qualifications:

Current and valid licensure as a Registered Nurse in the State of New York.

Ability to work effectively within a multidisciplinary team.
