We are seeking a compassionate and skilled Physical Therapist to join our Home Care team in Moab, Utah. In this full‑time role, you will evaluate, plan, and deliver individualized therapy to patients in their homes, supporting recovery, independence, and improved quality of life.
Responsibilities- Conduct physical therapy assessments and develop individualized care plans.
- Deliver safe, effective, evidence‑based therapy in the home setting.
- Collaborate with interdisciplinary teams, patients, and families.
- Document care according to regulatory and organizational standards.
- Educate patients and caregivers on therapy techniques and safe mobility practices.
- Current Utah Physical Therapist license required.
- Previous home health or acute care experience preferred, but not required.
- Strong communication, problem‑solving, and organizational skills.
- Compassionate, patient‑focused, and motivated to make a difference.
- Competitive compensation. Important:
