Day Shift - Monday - Friday, 8:00 AM - 5:00 PM (+ rotating weekends, holidays, + on-call schedule)

Responsibilities- Deliver Patient-Centered Home Care:
Provide high-quality nursing care, perform assessments, and develop and update individualized home health care plans in collaboration with patients, caregivers, and physicians. - Ensure Accurate Documentation and Compliance:
Maintain detailed clinical records, ensure compliance with regulatory standards, and participate in quality assurance activities to uphold best practices and patient safety. - Collaborate and Supervise:
Coordinate with interdisciplinary teams, communicate patient needs, supervise home health aides and LPN staff, and participate in education and training efforts.
Education:
- Graduate of State Board approved program for Registered Nurses.
Experience:
- One year nursing experience caring for similar patient population as to be assigned.
License(s)/Certification(s):
- Valid license as Registered Nurse in state where providing care.
- Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
- Mandatory Reporter:
Child + Dependent Adult Abuse. - Person Centered Care (PCC) course completion within first 12 months of hire + annual completion of competency validation activities.
- CPR:
Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
