Job Description

The Clinical Pharmacist is a licensed professional who is responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with company policies, professional standards, and applicable regulations. This role coordinates patient services with the Nursing Department, monitors patient progress toward goals, and educates patients, caregivers, staff, and other professionals regarding appropriate medication use.

Job Responsibilities
  • Determine suitability of patients for home care – Collects and organizes patient‑specific information, assesses suitability in accordance with Option Care policies, and evaluates medical condition, medication therapy, and prognosis with defined outcome goals.
  • Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans – Makes recommendations for medication therapy modifications, prevents medication‑related problems, identifies monitoring parameters, and completes nutritional assessments.
  • Modify pharmacy care plans based on monitoring data – Competent in pharmacokinetic principles, formulas, and laboratory value assessment.
  • Provide medication‑use education – Use patient education techniques to counsel patients, caregivers, and staff.
  • Ensure continuity of pharmaceutical care – Communicate patient information across care settings, prepare and dispense medications, calibrate equipment, manage labels, and comply with state/federal laws and USP standards.
  • Manage medication administration equipment – Program, maintain, and advise on equipment use and troubleshooting; participate in incident reporting and QAR documentation.
  • Respond to drug information requests – Formulate responses based on literature and attend CEU opportunities to maintain competency.
  • Manage investigational drug products – Follow regulations and policies, utilize support personnel, prioritize workload, and ensure accuracy of staff work.
  • Compliance with accreditation and safety requirements – Maintain confidentiality, observe legal and ethical guidelines, and comply with all regulations.
  • Maintain confidentiality and safeguard proprietary information – Observe patient and company confidentiality guidelines.
Basic

Education and Experience Requirements
  • Graduate of an accredited School of Pharmacy.
  • Current, active Pharmacy license.
  • One of the following: 1 year in sterile IV admixture procedures; knowledge of non‑sterile compounding, immunizations, and medication therapy management; completion of an ASHP‑accredited Pharmacy Residency; completion of an Advanced Pharmacy Practice Experience; or equivalent experience.
Basic Qualifications
  • Customer service experience with internal and external customers.
  • Basic skill in Microsoft Word, Excel, and PowerPoint (basic functions described).
Travel Requirements

Willing to travel up to 5% of the time for business purposes (within state).

Preferred Qualifications & Interests
  • Completion of an ASHP‑accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience.
  • Experience in home care, hospital, or clean‑room setting.
Management Requirements

At least 1 year of experience in indirect management of team members, including assisting with development, training, and assignment of work projects.
