Overview
Provides social work services to patients and families within assigned areas in order to meet identified psychosocial needs, enhance problem-solving and coping capacities, and facilitate timely discharge by arranging post-discharge services. The social worker assists with new Senior Nursing Facility (SNF) placement, addresses acute social determinants, and collaborates with all members of the healthcare team to ensure clear communication and care planning for all patients.
This role may include a sign-on bonus.

Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.
- Ability to coordinate multiple projects concurrently and establish priorities in a fast-paced acute care setting.
- Strong planning and organizational skills.
- Self-motivated with flexibility in hours and job duties.
- Ability to work effectively with diverse individuals and groups.
- Essential Functions
- Investigate cases admitted with "no insurance" or "self-pay" to identify whether insurance coverage exists and whether financial assistance is warranted.
- Evaluate patients for abuse/neglect and make appropriate referrals.
- Involve patient and family in discharge planning decisions.
- Keep the director/manager informed of problematic cases, especially those involving legal or risk management issues.
- Provide assistance and advocacy to patients in obtaining financial resources and government entitlements.
- Provide social work intervention to patient/families.
- Provide support and planning in cases of death, adoption, and fetal demise.
- Respond to referrals from healthcare team members to identify available facilities and services for case-specific needs during and post-hospitalization.
- Help identify Health Care Surrogate in accordance with the Health Care Surrogacy Act when appropriate.
- Assess patients who are uninsured/under-insured for IL Medicaid eligibility and/or Carle Financial Assistance Program eligibility. Provide direction or assistance with completing the application(s) where applicable.
- Complete PASRR assessments to facilitate discharge to extended care facilities.
- Complete mandated reports of suspected abuse and/or neglect of minors, disabled adults, and seniors; cooperate and collaborate with the investigating bodies for safe plans.
- Find, and if possible, arrange transportation for patients to medical appointments and/or home from the Emergency Department/Hospital.
- Educate about and connect patients with community resources to help meet their needs (e.g., non-skilled home help, meals/food, housing resources, charity programs, support groups).
- Provide empathic listening and emotional support to patients in person or over the phone.

Compensation And BenefitsThe compensation range for this position is $30.07 per hour - $51.72 per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. Actual compensation offered will depend on factors including experience, qualifications, location, licenses, shifts worked, and compensation model.
