• Join a team of physicians and advance practice providers supported by clinic support staff
• Monday through Friday 8 am - 5 pm
• Rotate between 3 nursing homes
• Call schedule requirement: 5 days per month including weekends, call handled by phone
• Average of 10 patients per day
