* Receive referrals from school sites regarding students with behavioral, mental, socioemotional concerns; assess behavior functioning utilizing information from teachers, individual history, and additional direct observations including but not limited to psychological, social work or other interdisciplinary team member observations to identify and recommend appropriate intervention strategies.
* Meet with identified students at school sites; confer with parents and conduct parent education trainings related to behavioral and socioemotional discipline issues.
* Attend and participate in meetings related to school site or individual student mental health needs.
* Conduct follow-up observations of student interactions and learning environments; assist classroom teachers to assess the effectiveness of behavior support systems to promote student success; provide feedback to teachers and administrators.
* Provide direct mental health services including counseling, consultation, evaluations, intervention coordination, individual or group counseling, coaching, and case management to students and families.
* Provides information and support to school staff regarding the school-wide learning environment and identified individual student needs.
* Serves as a liaison to other departments, agencies and community organizations including making referrals for assistance or support, coordinating activities, resolving issues and conflicts, and exchanging information to help meet student needs/goals.
* Represent IS/RO in external committee meetings working towards enhancing youth social emotional well-being and increasing access to youth services and resources in the community.
* Work with leadership to identify, plan and implement trainings and ways to increase best practices within the Intervention Specialist team.
* Prepare and maintain detailed case records, notes and reports related to assigned students and activities; monitor status of referrals from school sites; maintain confidentiality of sensitive and privileged information.
* Attend various meetings, workshops, and training to develop community partnerships and maintain current knowledge of developments in intervention for youth.
* Evaluate the effectiveness of intervention and student’s progress in resolving identified problems and moving towards defined objectives either independently or in collaboration with school staff. Maintains complete and accurate data on the efficacy of services and timely enter data and case note in the designated data and evaluation portal(s).
* Provide intervention services and short-term individual counseling related to interpersonal, substance use/abuse or other behavioral concerns. Provide consultation to parents, school personnel and other community agencies and personnel as a follow up to the intervention services implemented with the youth.
* Consistently engage in Reach Out’s equity work for the agency, with interaction at the DEI meeting and in a leadership capacity. All clients are served with trauma-informed care and equity lenses
* Work collaboratively with other departments at Reach Out on projects, events and assignments as requested throughout the Inland Empire.
* Participate in collaboratives, committees, meetings, events, workshops, and programs related to Youth and Family Wellness.
* Lead outreach efforts for the Youth and Family Wellness department.
* Provide guidance to coordinators.
* Assist with administrative tasks for the Youth and Family Wellness department.
* Fulfill regular assigned duties as determined by the Director.
* Identifying best interventions to meet the needs of individual youth.
* Familiarity with group dynamics and individual guidance based on needs.
