Brief DescriptionUpon orders of the physician, Social Worker identifies factors which influence a patient’s and/or family’s ability to respond to a medical therapy program. Through consultation and home visits, develops means to meet the goals of the plan of care. The Medical Social Worker is responsible for the coordination, delivery and documentation of Medical Social services and to report to the appropriate Team.
Job Duties / Responsibilities- Assesses patient’s social, emotional and safety factors related to his/her illness, taking into consideration the environment and present support system.
- Evaluates patient / caregiver decision-making ability.
- Assists patients / caregivers in recognizing deficiencies in meeting their care needs.
- Allows and encourages patient / caregivers to participate in identifying care needs if patient/ caregiver is capable.
- Identifies needs and establishes a plan of care.
- Conducts timely assessment as specified by Nurse Coordinator / Manager responsible for the case or per Agency protocol.
- Implements an action plan based on assessment of needs.
- Refers alleged patient abuse and/or neglect concerns or unsafe situations to proper authorities timely.
- Maintains current knowledge of resources available. Mobilizes available resources within existing time constraints.
- Provides feedback to referral resources timely.
- Involves extended family, with permission of patient/primary caregiver to assist in problem resolution.
- Completes documentation timely and accurately.
- Completes forms/electronic communications according to Agency policy procedure within established time frames.
- Documents follow-up communications with patients / families.
- Documents assessments, plans and resources contacts per established documentation protocols.
- Responds to and addresses documentation deficiencies within two (2) days of receiving notification of deficiency.
- Coordinates Care with inter-disciplinary team.
- Reports patient problems or changes in situation to Nurse Coordinator, Nurse Manager, physician and/or team members timely and appropriately.
- Attends, participates and documents case conferences as scheduled.
- Communicates with patients, significant others and interdisciplinary team members in a positive, respectful, and caring manner.
- Participates in Quality Improvement activities.
- Other duties, as assigned.
- Master’s Degree in Social Work required.
- Licensed to practice in New York State as a Medical Social Worker (LMSW or LCSW required).
- Two years of appropriate experience as a Medical Social Worker.
Community/home care experience is preferred.
