CSH Advance Practice Nurse

Overview

The provider is responsible for all care, including preventative care, diagnosis, management, and treatment to all patients and after hours call.

• Complete age appropriate evaluations and preventative care
• Routine age appropriate screenings as advised by American Academy of Pediatrics
• Provide routine immunizations as directed by American Academy of Pediatrics and Centers for Disease Control
• Management of acute and chronic illness
• Review of information from patient caretakers and patient
• Diagnostic testing as appropriate
• Prescription of pharmacological agents where indicated and management of prescribed medications
• Performance of procedures where indicated
• Referral and coordination of care with specialty providers when appropriate
• Management of care including home nursing orders, durable medical equipment, and plans of care for school
• Anticipatory guidance regarding growth, development, and medical conditions to caretakers
• Educate caretakers regarding pharmacotherapy, as needed
• Educate caretakers regarding procedures, as needed

• 20 hours primary care
• 4 hours training/chronic disease
