Community Support Services (CSS) / Supportive Housing

Community Support Services (CSS) / Supportive Housing provide services necessary to assist residents achieve mental health rehabilitative and recovery goals across a series of wellness domains including employment, education, housing, physical and mental health. Services begin with the development of an individualized recovery plan through a comprehensive personalized needs assessment. Persons served in this program learn and practice skills in order to achieve community integration and remain living independently in the setting of their choice.

Salary: Starting at $55000 - depending upon experience

Schedule: Monday – Friday / 9:00 am – 5:00 pm with (1) shift 10:00 am – 6:00 pm

Job Summary

As a Community LPN, you will join an evidence-based program and a dynamic team for the opportunity to learn and refine your clinical and engagement skills and accomplish your dream of helping people to make progress on their unique recovery journeys. The work is sometimes challenging and always rewarding by engaging with persons we serve and co-workers as we all learn, grow, and thrive.

Responsibilities
  • As part of the CSS team, provides nursing services, direct-support services, care management, education and primary health care to persons served.
  • Assures that medical needs are being addressed in the community.
  • Provides support and education to other team members in nursing/medical treatment and health care.
  • Nursing practice supervision is provided by the RN, or in group format quarterly by the Bridgeway Bridge-to-Wellness Medical Advance Practice Nurse.
Qualifications
  • High school diploma or equivalent, must have graduated from an accredited nursing training program and possess and maintain licensure, in good standing, in the State of NJ as an LPN
  • One year of technical nursing training supplemented with two years of paid work experience in community nursing
  • Valid driver’s license required with no more than one moving violation within the past 12 months (vehicle required)
