Autumn Lake Healthcare at Union is seeking a full‑time Physical Therapist to join a skilled nursing facility and provide outstanding, individualized care.

The Physical Therapist evaluates patients to identify needs and establishes an individualized treatment plan of care. By motivating patients during treatment, you will assist them in functioning optimally while promoting a healthy lifestyle through improvement of strength, flexibility, balance, and coordination. This role is ideal for therapists who enjoy being part of a cohesive group, are seeking thorough, continuous development, and would like the ability to involve in team decisions.

• Evaluate patients and establish individualized treatment plans.

• Collaborate with team members on assessment, documentation, and treatment planning.

• An active state Physical Therapist license is required.
• Unlimited employee referral bonus of up to $1,000 each.
• Option for PRN coverage at multiple locations with potential full‑time opportunities at additional sites.
• Direct hire position; no agency involvement.
• Open to PT candidates requiring H‑1B sponsorship, including current student visa holders, H‑1B transfers, and new H‑1B filings.

Am I guaranteed 40 hours?
While hours cannot be guaranteed, we typically offer a full caseload. Most facilities are nearby, allowing for additional hours at other locations.

Staff are eligible for performance reviews, which provide an opportunity for salary increases.

Our therapists often leave for other practice specialties; many stay as PRN but rarely move to competing skilled nursing facilities.

How does corporate support the therapists on the floors?
Regional Directors and many corporate members are therapists themselves. They understand the demands of Medicare and insurance guidelines and are actively present to collaborate with each team member, ensuring a well‑supported environment.

What type of mentorship or support do you provide for new graduates?
We offer a comprehensive mentorship program for new grads. You’ll be paired with an experienced therapist who will guide you through documentation, evaluations, treatment methods, and more. As you gain confidence and work independently, ongoing support is available.

Salary
: $45 – $51 per hour
