Essential Duties and Responsibilities:
- Direct and coordinate the operations of all Substance Abuse/Mental Health Programs including providing supervision to MSW intern students and Support Coordinators under the DDD program
- Completion of the NJSAMS and EHR Intake documentation for all incoming clients.
- Provide individual, group, and family counseling to assigned caseload.
- Completion of Substance Abuse/Mental Health disorder biopsychosocial/Behavioral assessments, client’s education, clinical case management and treatment plans for all clients based on research, clinical experience, and client histories.
- Interview clients, review records, and confer with other professionals to evaluate individuals' mental and physical condition and to determine their suitability for participation in a specific program.
- Ensures the policies and procedures that guide and support the provision of services are implemented and that recommendations to improve performance are communicated to administration.
- Provide clients or family members with information about addiction issues and about available services or programs, making appropriate referrals to detoxification or higher level of care when necessary.
- Intervene as advocate for clients in order to resolve emergency problems in crisis situations.
- Referrals to the Social Services agencies in community such as (shelters, welfare).
- Maintain client records and ensure documentation is in compliance with DMHAS and agency requirements and standards.
- Schedule and organize treatment team meetings and keep updated records and files of all programs.
- Provide quality case management and maintain phone regular contact with the individual, natural supports, and social service agencies (when applicable) to provide updates on treatment and progress.
- Follow progress of discharged patients in order to determine effectiveness of treatments.
- Attendance and participation in supervision with clinical supervisor/director.
- Attendance and participation in case conferences and case consultation with interdisciplinary staff.
- Work with other agencies and promotes good community relations through involvement in community events and other marketing events. Implement specific measures for program growth.
- Attendance and participation in scheduled in-service trainings, meetings, and other trainings required by DMHAS and the agency.
- Assist with the hiring process of all clinical staff
- Participate in on call schedule
- Other duties as assigned.
- Master’s degree in Social Work or related field
- Professional demeanor and appearance
- Proficient skills to promote excellent client relations and customer skills
- Must be knowledgeable of Excel, Word, NJSAMS, EHR navigation, and general office systems
- Excellent oral/written communication, excellent organization/time management skills and presentation skills.
- Knowledge of the New Jersey levels of care and treatment is preferable.
- Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities
- LCADC, LSW, LCSW certification strongly preferred
