Social Work Assistant – Integrated Triage & Assessment Services

We are currently looking to recruit a permanent social work assistant to join our Porth Gofal Integrated Triage and Assessment Team, working with individuals and their families within hospital and community settings, with a focus on recovery and rehabilitation.

This is an exciting role within the service, providing the opportunity to undertake assessments and implement packages of care to support our organisation’s aim of enabling citizens and family resilience.

The role is one of constructing and implementing care and support plans that are person այսօր‑centre and outcome‑focused and that respond flexibly to changing needs.
