We are looking for a compassionate and enthusiastic Practice Nurse to join our friendly and supportive clinical team.
We are a well-established practice serving around 6,200 patients, with an excellent reputation for high-quality, personalised, patient-centred care. Continuity of care and the culture of traditional family medicine are strong values in the practice, and support our "cradle to grave" relationship with patients.
We would prefer applications from applicants who have previous experience of working in a GP practice, but we would also welcome applications from nurses with a background in Community Nursing.
The role will cover all aspects of practice nursing, including:
- Baby immunisations
- Cervical screening

Main duties of the jobProvide safe, evidence-based nursing care to patients of all ages.
Carry out baby and childhood immunisations in line with NHS vaccination schedules.
Manage patients with chronic illnesses such as diabetes, COPD and asthma.
Undertake long‑term condition reviews, including health checks and lifestyle advice.
Contribute to meeting QOF and Enhanced Services targets.
Maintain accurate and timely patient records using clinical IT systems (e.g. EMIS).
Work collaboratively within the practice nursing team, GPs, and other healthcare professionals

We foster team working across the practice, from GPs that have an 'open door' philosophy forstaff queries, to our practice nurses having dedicated admin and nurse meetingtime.
Job responsibilities- To assist medical personnel in the care of practice patients to include treatment, preventative care, screening and patient education. The post will also include some administrative duties.
- To act as a focal point of communication between patients, doctors, nurses and other members of the primary health care team
- To maintain a safe and effective level of practice and to bring any deficits in training to the attention of the nursing team leader so these can be addressed.
- To act in a professional manner at all times and adhere to practice policies, protocols and guidelines.
- To work as an effective team player
- To provide continuing care to patients across the whole practice population.
- To provide health maintenance and health promotional services to the well, offering educational supportive setting for patients to explore their health problems.
- Registered first level nurse
- Clinical skills cervical cytology
- Immunisation and vaccination
- Wound care experience including compression bandaging
- Chronic Disease Management
- Travel Health
- Minimum 2 years post‑registration experience
- At least 1 year primary or community nursing experience
- Aware of accountability of own role and other roles in a nurse‑led service
- Ability to identify determinants on health in the local area
- Knowledge of public health issues in the local area
- Awareness of local and national health policy
