Practice Nurse

The ideal candidate will be a qualified Practice Nurse with experience in a primary care setting.

The successful candidate will be expected to contribute to the delivery of the practices Quality and Outcomes Framework (QOF) programme and support the wider clinical team in maintaining high standards of patient care.

We are an active teaching practice with GP trainees, junior doctors and student nurses. The successful candidate will be supported by a GP mentor and will have access to regular teaching and development opportunities where required.

Key responsibilities will include:

• Protocol-led nurse triage (both face-to-face and telephone)
• Delivery of routine and chronic disease management clinics
• Participation in preventative care and health promotion
• All aspects of general practice nursing

Job Summary

The post holder will provide assessment, screening and treatment services to patients, alongside health education and health promotion advice.

The role will involve delivering high-quality nursing care within a primary care setting, supporting the wider clinical team and contributing to improved health outcomes for the practice population.

Main Responsibilities

• Provide assessment, screening and treatment services for patients
• Deliver health education and preventative health advice
• Support chronic disease management and preventative care programmes
• Manage treatment room supplies and equipment
• Undertake pathological specimen collection and other investigatory procedures
• Ensure accurate clinical notes and records are maintained on the clinical system
• Complete all necessary documentation associated with patient care and registration
• Maintain statistical information required for regular and ad-hoc reports
• Attend and contribute to practice and clinical meetings
• Contribute to the development of practice strategy, policy and clinical standards
• Work collaboratively with the wider multidisciplinary team

Westrop Medical Practice is a member of the Brunel Health Group Primary Care Network, a collaboration of 14 practices working together to deliver services at scale for a population of around 150,000 patients. Through Brunel we support a range of innovative services including paramedic home visiting, enhanced clinical roles and large-scale vaccination programmes. Brunel is also one of the largest primary care research groups in the South West.

To support the clinical team in delivering high-quality patient care by providing assessment, treatment, screening, preventative care and health education to registered patients.

The post holder will work collaboratively with the multidisciplinary team to deliver safe, effective and patient-centred care in accordance with practice protocols and national clinical guidance.

The Practice Nurse will:

• Assess patient needs and provide appropriate nursing care either independently or in conjunction with a GP or other healthcare professional
• Deliver treatment room services including wound care, venepuncture, ECGs and other clinical procedures
• Undertake immunisations and vaccinations
• Assist with the management of patients with long-term conditions
• Carry out screening programmes including cervical cytology
• Provide health promotion and lifestyle advice to patients
• Recognise and respond appropriately to common childhood illnesses
• Evaluate the outcomes of care and adapt care plans accordingly
