Weare seeking a dynamic and motivated Nurse Practitioner to provide high quality clinical assessment, diagnosis, treatment, and evaluation for patientspresenting with a wide range of acute and long term conditions. The successful candidate will work autonomously as well as collaboratively with our multidisciplinary team.

Main duties of the job
  • Provide advanced clinical assessment, diagnosis, and treatment within your scope of practice
  • Manage patients presenting with undifferentiated, undiagnosed conditions
  • Prescribe medications and manage treatment plans
  • Support the practices urgent care/on-the-day demand
  • Work closely with GPs, ANPs, Practice Nurses, HCAs, and other allied health professionals
  • Maintain accurate and timely clinical records

Job responsibilities

Job Summary

The post holder will provide same day assessment and care for patients presenting with urgent or acute clinical needs. This includes both telephonetriage and face to face consultations. They will independentlyassess, diagnose, treat, and manage patients presenting with minor illness,minor injury, or acute exacerbation of chronic conditions, referring to investigations or external services when required.

The Nurse Practitioner will work autonomously within their professionalcompetencies, seeking support from the duty GP as appropriate. Excellent clinical judgement, prioritisation skills, and safe practice are essential tothe role

Qualifications
  • Qualification in physical assessment and diagnosis
  • Evidence of ongoing professional development
  • Teaching/mentoring certificate
  • Qualification in management of chronic disease eg COPD
Experience
  • Experience in autonomous management of minor illness and injury
  • Strong clinical decision making, planning, coordination and evaluation skills
  • Up to date knowledge of clinical developments and professional issues
  • Experience managing chronic disease
  • Audit or quality monitoring experience
