Clinical Pharmacist
We are looking for an enthusiastic Clinical Pharmacist to join our supportive team at The Tutbury Practice.
This is an exciting opportunity to play a key role within our multidisciplinary primary care team, helping to improve patient outcomes through high-quality medicines optimisation and patient-centred care. The successful applicant will work closely with GPs, nurses, and other healthcare professionals within the practice and across our Primary Care Network (PCN).
The Clinical Pharmacist will work within their professional boundaries to provide expert advice on medicines management, support patients with medication reviews and long-term conditions, and contribute to improving the safety and effectiveness of prescribing across the practice.
The role includes direct patient contact through clinics, medication reviews and collaborative care with the wider healthcare team. You will also act as an important link between primary and secondary care to ensure safe and effective use of medicines.
As part of this role, applicants must hold an Independent Prescribing qualification and be confident prescribing safely within a primary care setting. You will be supported through regular clinical supervision and mentorship within the practice and the wider Primary Care Network to help you continue developing your skills and professional interests.
Main duties of the jobYour role will include:
- Providing structured medication reviews for patients, particularly those with complex medication regimens or multiple long-term conditions.
- Supporting management of patients with complex polypharmacy, including elderly and frail patients, care home residents, and those with conditions such as COPD, asthma, learning disabilities or autism.
- Helping patients get the most benefit from medicines by improving understanding, adherence and self-management.
- Supporting medicines optimisation and antimicrobial stewardship within the practice.
- Acting as a key contact for medication-related queries from patients and staff.
- Delivering clinics and consultations for patients with long-term conditions where appropriate.
- Working closely with community pharmacists, hospital teams and other healthcare professionals to support continuity of care.
- Supporting safe prescribing systems including repeat prescribing and monitoring of high-risk medications.
- Contributing to quality improvement including prescribing audits and adherence to NICE guidance and QOF requirements.
- Supporting vulnerable patients and those at risk of hospital admission through proactive medication reviews.
- Contributing to public health initiatives such as vaccination campaigns and health promotion activities.
- You will also be encouraged to contribute to service development and share expertise with colleagues as we continue developing our pharmacy services.
The Tutbury Practice is a rural based practice with a population of around 8500 patients.
Our team consists of 2 GP partners, 3 salaried GPS, 1 registrar, 1 Nurse Practitioner, and 1 Nurse Manager, 1 Deputy Nurse Manager with awell-established nursing team complemented by a friendly and established adminand reception team. The CQC rating is good.
The Clinical Pharmacist will:
- Work as part of a multidisciplinary team in a patient-facing role to assess and manage patients using their expertise in medicines.
- Undertake clinical medication reviews
, particularly for patients with complex polypharmacy or long-term conditions. - Support the care of patients in care homes and those with multiple comorbidities.
- Provide specialist advice on medicines to clinicians, patients and carers.
- Support safe and effective prescribing through medicines optimisation and quality improvement.
- Support and improve repeat prescribing systems within the practice.
- Respond to medication queries from patients and members of the practice team.
- Monitor patients on high-risk medicines and support appropriate therapeutic drug monitoring.
- Work with community and hospital pharmacy teams to support safe transitions of care.
