Location: City

We are currently working with a client in Liverpool who is looking to recruit an experienced Social Care Assessor to join their Adult Access Team.

As a Social Care Assessor, you will be responsible for assessing individuals and, where necessary, providing short-term support or reablement to promote independence. Where longer-term intervention is required, you will complete robust assessments to support a smooth transition into Locality Teams for ongoing support.

You will work with adults aged 18+ and manage a varied caseload with the support of supervision and guidance, ensuring all work is carried out in line with relevant legislation, policies, and procedures.

Key Responsibilities:

  • Manage and prioritise a varied caseload effectively
  • Complete Reablement, Needs, Risk, Mental Capacity, and Best Interest Assessments, as well as reviews
  • Commission appropriate services including domiciliary care, day placements, carers support, assistive technology, and minor adaptations
  • Work collaboratively within a Multi-Disciplinary Team
  • Attend daily huddles and participate in duty responsibilities
  • Represent the Adult Access Team at external meetings when required
  • Apply the Care Act's Prevent, Reduce, Delay principles to promote independence and wellbeing

Key Requirements:

  • Previous experience working within Adult Social Care
  • Experience completing strength-based assessments
  • Knowledge of relevant legislation including the Care Act
  • Ability to manage a varied caseload effectively
  • Experience working within a Multi-Disciplinary Team
  • Strong communication and report writing skills
  • Ability to promote independence and person-centred outcomes

Contract Length: 3-6 months with a view to extend to an ongoing contract.
Pay Rate: 19- 25
Working pattern - 37 hours per week, onsite.
