Overview
Hywel Dda University Health Board is recruiting a Band 6 Mental Health Practitioner to deliver a 24/7 telephone mental health triage and assessment service for individuals presenting with mental illness, distress or acute crisis. The role will operate across Prince Philip Hospital, Llanelli and Withybush General Hospital, Haverfordwest, and will support the transformation of the Mental Health Single Point of Contact service through the National 111 network.
Responsibilities- Provide high‑quality telephone triage and assessment to all‑age individuals, ensuring safe and appropriate clinical decisions.
- Lead clinical shifts with a multidisciplinary team of Well‑being Practitioners, Health Care Support Workers and Peer Workers, acting as escalation point and decision maker for risk assessment.
- Support the Clinical Lead in developing and delivering the innovative Single Point of Contact service.
- Participate in 24/7, out‑of‑hours and weekend working patterns to meet service demand.
- Engage in continuous professional development and contribute to team training and induction programmes.
- Advocate for compassionate, preventive, equal and resilient care, aligning with the board’s core values.
- Band 6 Mental Health Practitioner or equivalent clinical qualification.
- Extensive experience in mental health triage, assessment and clinical decision making.
- Strong leadership and teamwork skills, with a proven ability to work within multidisciplinary teams.
- Excellent communication skills, capable of providing clear advice and guidance to families, carers and partner agencies.
- Willingness to work flexible hours including evenings, nights and weekends.
Welsh is desirable but not mandatory for this post; all applicants are encouraged to apply in Welsh or English and the Health Board offers language support programmes.
Service locations include Prince Philip Hospital (Llanelli) and Withybush General Hospital (Haverfordwest). The work hours are 24/7, 7 days a week.
