OverviewAs an Occupational Therapist you will provide professional support, train team members, and act as an expert advisor on equipment and decision‑making. You will conduct strengths‑based assessments and design innovative reablement plans tailored to individual needs, encouraging independence and minimising long‑term care needs. With the autonomy to manage your time and schedule, you’ll be able to provide bespoke care to individuals in your diverse caseload, ensuring their unique needs are met with compassion and expertise.

- Manage a busy caseload including complex and challenging cases.
- Provide support and guidance for the management of complex and challenging cases.
- Re‑prioritise work according to individual customer and service needs.
- Undertake strengths‑based occupational therapy assessments, including assessment of mental capacity when appropriate, and adapt your approach to the individual needs of the customer.
- Design and implement innovative reablement plans for people living in complex situations to enable customers to meet their goals and reduce the need for ongoing support.
- Coordinate the customer’s reablement journey, in collaboration with the customer and/or family members, including regular contact with the customer, team members, and relevant partners to evidence decision making, plan interventions, respond to change, and evaluate progress.
- Demonstrate competence in complex risk assessment and positive risk‑taking, work with customers to manage risk, and record agreed actions and strategies.
- Support other team members with risk management and liaison with family members and support staff on options to encourage independence and safe practice.
- Occupational Therapy qualification.
- Minimum of two years post‑qualification experience.
- Experience working with customers with complex occupational needs and contentious situations. < protected>
