RN Case Manager

We're looking for a dedicated RN Case Manager who is passionate about patient advocacy, care coordination, and ensuring individuals receive the right care at the right time. This role offers a structured weekday schedule while still providing the dynamic, hands-on nature of case management - including care planning, discharge coordination, and occasional in-home or hospital evaluations.

What You'll Do

In this role, you will:
Implement and evaluate individualized patient care plans
Review clinical information to assess medical necessity and appropriateness of services
Support prospective, concurrent, and retrospective case evaluations
Advocate for patients and families throughout the care journey
Coordinate discharge planning to ensure smooth transitions of care
Assess complex outpatient needs and collaborate with multidisciplinary teams
Conduct in-home or hospital evaluations when appropriate
Maintain accurate and timely documentation
Help ensure services are delivered in the most effective and cost-conscious manner

What We're Looking For
Associate's Degree or higher in Nursing
Active Florida RN license
Case Management Certification (or ability to obtain within one year)
Minimum of 2 years of Medical Case Management experience
BLS Certification
Valid Florida Driver's License
Strong clinical knowledge and understanding of current healthcare practices
Excellent communication, organization, and patient advocacy skills
Comfortable working across care teams and coordinating services
Proficiency with Microsoft Office tools

Flexibility is important - occasional evenings, weekends, or holidays may be required based on patient needs.
