Admissions Nurse (LPN) – Weekdays and Rotating Admissions On-call

Schedule: Monday through Friday

The Admissions Nurse (LPN) is responsible for coordinating and completing the clinical admissions process for new residents, ensuring accurate assessments, documentation, and a smooth transition into the long-term care facility.

• Complete initial nursing assessments for new admissions
• Review hospital and referral documentation for accuracy and completeness
• Coordinate admissions with nursing, therapy, social services, and administration
• Verify physician orders and initiate care plans upon admission
• Ensure admission documentation meets regulatory and facility requirements
• Communicate with families, hospitals, and referral sources as needed
• Assist with resident room readiness and admission flow
• Dealing with Referrals
• Uploading in PCC
• Tours
• Rotating on call with our Marketer
• Perform other duties as assigned

• Active LPN license in good standing
• Long-term care or skilled nursing experience preferred
• Strong assessment, documentation, and organizational skills
• Ability to manage multiple admissions and priorities
• Professional communication and attention to detail
• Reliable attendance for a Monday–Friday schedule
