Certified Nursing Assistant (CNA)

Providence Park Rehabilitation and Skilled Nursing in Tyler, TX offers a rewarding career as a Certified Nursing Assistant (CNA) at our long‑term and rehabilitative care facility. We provide an 8‑hour shift, personal care, assistance to the charge nurse, and exceptional service as part of a multi‑disciplinary team.

• Provide personal care to residents safely and comfortably.
• Promote independence for residents.
• Inform the supervising charge nurse of residents’ status at shift beginning and end.
• Contribute to resident care plans based on conditions/needs.
• Assist with care plan implementation per established policies.
• Observe and report residents’ conditions; document observations as required.
• Follow safe lifting and transfer policies.
• Perform moderate to heavy lifting.
• Ensure equipment is clean and functional.
• Perform incidental housekeeping or maintenance to keep a clean, hazard‑free environment.
• Other duties as assigned.

• State certification as a nursing assistant.
• Familiarity with duties of a certified nursing assistant.
• Highly developed observation skills.
• Excellent communication and interpersonal skills.
