Geriatric Home Health LVN

Amcare Pro Home Health is a mid‑size home health care company with offices in East Texas and Dallas. We are currently interviewing for a PRN LVN Field Nurse to cover the Tyler, Chandler, Flint, Bullard, Lindale and surrounding areas.

We are looking for a Home Health LVN to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse. This role supports the delivery of safe, high‑quality patient care through routine nursing visits, treatment administration, patient education, and timely documentation. Previous Home Health Care experience is a must. Familiarity with electronic charting (KINNSER/AXXESS) is a plus.

Responsibilities:

• Provide skilled nursing care to home health patients under the supervision of an RN
• Perform routine nursing visits including vital signs, medication administration, injections, and treatments
• Assist with wound care and dressing changes per physician orders
• Monitor patient condition and promptly report changes to the supervising RN
• Educate patients and caregivers on medications, treatments, and safety
• Accurately document all visits and care provided in the agency EMR
• Ensure compliance with Medicare, Texas regulations, and agency policies
• Coordinate visits and communicate effectively with the clinical team
