Registered Nurse - Caregiver

Position: Registered Nurse - Caregiver Support
The Central Alabama VA Medical Center-Montgomery is seeking a Nurse - Caregiver Support Case Manager to fill the role in Nursing, Whole Health Outcomes (WHO). The position is assigned responsibility for providing comprehensive, high quality, cost effective healthcare services to patients who are rendered care at the Central Alabama VA and by/through community providers.

The Caregiver Support Program (CSP) includes the Program of Comprehensive Assistance for Family Caregivers (PCAFC) and the Program of General Caregiver Support Services (PGCSS) within the Veterans Health Administration (VHA).

Major duties and responsibilities include:

* Conducts interim and annual home visits to caregivers and Veterans approved in the program and provides assessments via phone and clinical video telehealth.

* Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.

* Demonstrates performance and leadership that is broad enough to improve the care for a group of patients.

* Provides peers with formal or informal constructive feedback for improvement.

* Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.

* Fosters a safe and supportive environment conducive to the professional development of health care professionals.

* Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.

* Responsible for the documenting broad and complex outcomes at the program or service level, that can be demonstrated at any organizational level within a facility.

* Other duties as assigned.

Pay:
Competitive salary, regular salary increases, potential for performance awards
