*** Provide one-on-one skilled nursing care to patients in their homes (e.g., wound care, catheter care, medication administration, symptom management, etc.)
* Provide skilled observation, assessment, and evaluation
* Provide patient and family/caregiver support and education
* Practice cohesive and collaborative care with the healthcare providers
** Requirements and Qualifications
*** Current WA
** Licensed Practical Nurse (LPN),
** or before first day of work
* Prior Home Health and/or Hospice experience preferred
* Ability to work independently and demonstrate critical thinking
* Current BLS certification from AHA
* Be a licensed driver with an insured automobile in good working order
