JOB SUMMARY
This role is supported by time-limited funding. While continuation beyond the current funding period is not guaranteed, the organization is committed to pursuing ongoing funding opportunities.
Primary Responsibilities- Offer rapid response (e.g., a phone call within 48 to 72 hours) when a family has requested assistance.
- Listen and provide confidential emotional support.
- Help families with goal-setting.
- Empower families to make informed choices that are right for them.
- Assess families’ needs and strengths.
- Collect assessment and referral information.
- Provide peer support in one-on-one and group settings.
- Role model and coach families how to identify and access the supports they need.
- Refer and link families to community resources.
- Help families understand the mental health evaluation, diagnosis, and treatment process.
- Discuss options in an unbiased and culturally sensitive manner.
- Explain processes, rights and responsibilities, and timelines for services.
- Offer guidance with forms and paperwork.
- Support transition, safety, and reentry planning.
- Identify relevant benefits programs (e.g., Medicaid, SNAP); support enrollment as appropriate.
- In collaboration with the Community Alignment Manager and Prevention Community Educator, engage in school or community outreach and awareness events.
- Identify and nurture relationships with community resources, services and providers that can offer system support to families in a timely and appropriate manner as referred by PCCT. Maintain an updated list of community resources to support PCCT Family Connects and other PCCT staff as they connect families with resources, as needed.
- Coordinate FRC meetings, workshops and trainings for delivering services in compliance with established guidelines.
- Responsible for the purchase and inventory of consumables and supplies to keep the FRC stocked and organized.
- Work with the Community Alignment Manager to promote the Family Resource Center/PCCT by communicating with referral sources and groups that interact with families, and soliciting support from community leaders to publicize the program and provide support for families.
- Create and liaise with a Parent Advisory Committee for the Family Resource Center to provide ongoing needs assessment and continuous quality improvement.
- Attend trainings as required, including Standards of Quality for Family Strengthening & Support, Developing and Sustaining Effective PACs, and technical sessions.
- Demonstrates the agency’s values of compassion, respect, learning, adaptability and achievement.
- Other duties as assigned by the Community Alignment Manager.
- Ability to read, write, and speak English and Spanish
- Firsthand experience navigating systems on behalf of child, youth, or young adult with health challenges and working with families.
- Familiarity with the resources in your community and region.
- Understanding of relevant systems, services, and processes (e.g., TANF, Medicaid enrollment).
- Strong interpersonal skills and empathy.
- Understanding of and ability to maintain family confidentiality.
- Familiarity and ability to work effectively with families of diverse racial, ethnic, socioeconomic, and cultural backgrounds (e.g., LGBTQ, military).
- Strong written and spoken communication skills, including familiarity with relevant languages for your region.
- Ability to collaborate and network with community organizations and groups.
- Active involvement in other community councils, advisory boards, or parent and family groups.
- Formal training and/or work experience within the social service/case management field or training specific to navigating community service systems
- Ability to lift 25 lbs.
- Bachelor’s degree in the social services or other relevant field.
- Experience in human services in the public sector, skill in interacting with human service providers and agency leaders, and an understanding of the role of evaluation in social services.
