The Home Care LPN is responsible for delivering nursing care and assisting residents with daily living activities in a home setting. They work under the supervision of a registered nurse (RN) or physician and collaborate with a healthcare team to ensure patients receive quality care.
Supervisory Responsibilities:

Essential Functions/Duties/Responsibilities:
Patient Care:
* Prepare medication sets, administer medications and treatments as prescribed.
* Monitor and record patients' vital signs and health status.
* Assist with personal care activities, including bathing, dressing, and grooming.
Health Monitoring:
* Observe and report changes in patients' conditions to the supervising nurse.
* Perform basic health assessments and document findings.
Care Coordination:
* Collaborate with RNs, physicians, and other healthcare professionals to develop and implement care plans.
* Educate patients and families about health conditions and care procedures.
Documentation:
* Maintain accurate and up-to-date patient records, including care provided and any changes in condition.
Patient Support:
* Provide emotional support and companionship to patients and their families.
* Assist with mobility and physical therapy exercises as directed.
Hygiene and Safety:
* Ensure a clean and safe environment for patients.
* Follow infection control procedures and safety protocols.
Equipment Management:
* Use and maintain medical equipment as needed, such as oxygen tanks and feeding tubes.
On Call Rotation:
* Participate in the after-hours/on-call rotation in order to handle immediate and/or emergency scheduling needs.

Education and Experience:
Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state of Oklahoma.
Minimum 1 year experience in nursing preferred.
Minimum 1 year experience in Home Health preferred.
Physical Requirements:
Moderate amount of physical exertion including standing, walking, sitting; and occasional requirement for twisting/turning, bending at the waist, lifting above head and shoulders, and routine lifting and transferring/repositioning of patients with loads of up to 35 lbs.; and pushing wheelchairs with loads of up to 300 lbs.
Normal levels of hearing and vision, including both near and far seeing, color discrimination, field of vision and depth perception and ability for ordinary levels of communication, both written and verbal
Frequent requirement for normal level of eye-hand coordination
Moderate use of office equipment, online scheduling software, and smart phone
Ability to drive the Home Care, resident, or personal vehicle to transport residents or run errands, as necessary
