About the Role
We are seeking a compassionate and experienced Nurse Practitioner (Family or Adult-Gerontology NP) or Physician Assistant (PA) to join our dedicated post-acute care team. This position plays a crucial role in delivering comprehensive, patient-centered care across Skilled Nursing Facilities (SNFs), Rehabilitation Hospitals, and Long-Term Acute Care Hospitals (LTACHs).
You will collaborate closely with attending physicians, nurses, and rehabilitation specialists to ensure coordinated, high-quality medical care for patients transitioning through post-acute recovery.
Key Responsibilities- Conduct comprehensive patient assessments and review medical histories
- Diagnose and manage acute, chronic, and complex medical conditions
- Order, interpret, and follow up on diagnostic tests (laboratory, imaging, etc.)
- Develop and implement individualized treatment and care plans
- Prescribe medications and manage ongoing pharmacologic therapy
- Partner with attending physicians, rehab therapists, and nursing staff to optimize outcomes
- Educate patients and families on conditions, treatment plans, and preventive health
- Maintain accurate and timely documentation in the EMR system
- Promote health awareness and disease prevention across the care continuum
- Active and unrestricted FNP, AGNP, or PA license in the state of Oklahoma
- Proven experience in post-acute care, hospitalist, or internal medicine environments
- Strong clinical background managing patients with multiple comorbidities
- Skilled in communication and interdisciplinary collaboration
- Knowledge of healthcare regulations and EMR documentation standards
- Compassionate, adaptable, and dedicated to providing high-quality patient care
- Full-Time: Monday to Friday, 8:00 AM to5:00 PM
- Weekend Coverage: Limited and rotational, as required
- Range: $120,000 to $140,000 per year (commensurate with experience)
